What Causes Gynecomastia (Male Breasts)?

Gynecomastia is caused by hormonal imbalance in men. When estrogen, prevalent in women, is produced more than testosterone, it results in lump-like mass on either or both breasts. It can occur during the early stages after childbirth, puberty or old adulthood.

The hormonal imbalance between estrogen and testosterone can be more or less. When the imbalance of the two hormones is more, the breast will become larger than when the imbalance is less.

Dr. Tarek Bayazid, one of the leading plastic surgeons in Dubai, says there are other factors that cause Gynecomastia.

Can Genetics Cause Gynecomastia?

The process of having excess syndrome where estrogen is produced in large quantities in men and women is called aromatase. This excess syndrome is the chief contributor to gynecomastia.

The rearrangement of genetic materials that involve the CYP19A1 gene causes excess syndrome known as aromatase. The CYP19A1 gene is responsible for making the enzyme aromatase. This enzyme converts hormones called androgens which help develop male sexual growth into different forms of estrogen. The estrogen in men helps in regulating bone growth.

This rearrangement involving the CYP19A1 gene can sometimes change and cause an increase of aromatase production. Excess production of aromatase leads to gynecomastia in men.

Can Food Consumption Cause Gynecomastia?

One of the reasons why men have man-chest is poor dieting. The food you eat plays a role in developing your body. When you eat the same type of food excessively there will be repercussions.

In this case, consuming food with high fat levels, high carbohydrates and high sugar will eventually lead to a lot of weight gain. The high weight gain will cause excess fat causing your body to bulge. Some of that fat will be deposited in your breast tissues.

Can Gynecomastia be Inherited?

Sometimes gynecomastia in men is inherited in an autosomal dominant pattern. It means a copy of CYP19A1 gene in each cell that undergoes genetic rearrangement is enough to cause a disorder. The disorder results in a mutation of the gene.

The affected person can inherit the mutation from one of the affected parents. Other occurrences of this mutation are a result of new genetic changes that happen to people with no history of the disorder.

Can Diseases Cause Gynecomastia?

On rare occasions, some diseases can cause man-chest. These diseases include:

  • Liver disease – liver problems leading to hormonal changes can cause gynecomastia.
  • Kidney failure – around 50% of people going through dialysis suffer from gynecomastia because of hormonal changes.
  • Hypogonadism – where testosterone production is reduced by conditions such as Klinefelter syndrome.
  • Tumors – Tumors related to the testes, adrenal gland, and pituitary gland cause an imbalance of the male-female hormones.

Gynecomastia As a Result of Medication

Dr. Tarek observes that when we become sick, the medicine used for treatment should boost or suppress some hormones to facilitate quick healing. Unfortunately, as medicine works to resolve one issue, it can allow another to develop.

One such hormone is Prolactin. It is a natural hormone found in both men and women. But when prolactin is excess in your body, you will suffer Hyperprolactinemia leading to gynecomastia.

Some of the medicine that causes gynecomastia are:

  • Haloperidol
  • Risperidol
  • Chlorpromazine
  • Fluphenazine
  • Methyldopa and more

There are other medicines that trigger more estrogen production when used in men. As mentioned previously, high estrogen is one of the causes of gynecomastia in men.

Some of these medicine that may cause high estrogen (estrogen-like effect) production are:

  • Diazepam (Valium)
  • Digoxin (Lanoxin)
  • Phenytoin (Dilantin) and more

Androgens are important hormones in men’s bodies. One of the androgens is testosterone. Testosterone and dihydrotestosterone (DHT) are part of androgens, responsible for keeping estrogen production in check.

Some medicines interfere with the function of testosterone and DHT hindering them from blocking the effects of estrogen. When estrogen is not well restricted by androgens, you will likely suffer from gynecomastia.

Some of these medicines that interfere with androgens are:

  • Ketoconazole
  • Cimetidine (Tagamet)
  • Finasteride (Proscar)
  • Spironolactone (Aldactone)
  • Prostate cancer medicine like Bicalutamide

It is important to note that not all the medicines mentioned will cause gynecomastia in men. The same medicine may cause man-chest on some. But it may not do so on others.

Winding Up

All men suffering from gynecomastia would want to do something about it. Its effect usually weighs heavily on men such that some would do anything to overcome it.

Dr. Tarek encourages men who suffer from gynecomastia to seek help in finding a way to resolve it, including gynecomastia surgery. He reiterates that mostly, man-chest are a result of circumstances beyond your control.

As much as you may suffer a dip in self-esteem, you are not alone. There are many who have tread on the same road and have found relief.

Frequently Asked Questions

Other than the pronounced size of the breast, some men might experience a bit of pain and tenderness. Other men get irritations that cause itchiness, chafing and infections. In rare cases, the nipples of some men may discharge.

There are no definitive ways of preventing it, but doctors recommend that:

  • You shouldn’t drink excessive alcohol
  • You should keep off illegal drugs
  • Avoid using steroids

The combination of fat and tissue contributes to the increased breast size. Exercising can reduce the fat in the breast but not the glandular tissue. The only way to get rid of the tissue is by undergoing a surgery.

As you get older, your testosterone levels also decline. This decline results in low libido (sex drive), erectile dysfunction and a low sperm count.
